Has Your Tooth Moved? This Could Be Why (and It Is More Common Than You Think)

A few weeks ago, a visibly worried patient entered my office. She said: “Doctor, one of my teeth is moving, but it does not hurt at all… is that normal?”I looked at her with the same seriousness with which I write to you today: when a tooth moves, there is no time to lose.
I have been a dentist in Santo Domingo for more than 20 years, and if there is one thing I have learned, it is this: teeth should not move once the growth stage has ended. If something is shifting, there is a reason… and finding it is urgent.
Although most people associate loose teeth with blows or falls, in my clinical experience I have seen that more than 80% of cases are due not to trauma, but to silent causes that gradually wear away the tooth's support without painful signs at first.
The most common causes include:
1. Periodontal disease (the most common)
When tartar and plaque build up beneath the gums, a chronic infection develops that destroys the bone supporting the tooth. This disease can progress for years without pain, and by the time the patient notices the tooth moving… bone loss has already occurred.
At our dental clinic in Santo Domingo, we see daily cases of periodontitis ranging from moderate to severe that could have been stopped in time with a simple evaluation.
2. Sleep bruxism (clenching your teeth without knowing it)
Many patients do not know that they grind or clench their teeth while sleeping. This causes microfractures, overload on the tooth root, and, over time, mobility. Bruxism is a silent enemy of your smile and can affect even young people with no other apparent problem.
3. Malocclusion or bite problems
When teeth are not properly aligned, some receive more pressure than normal. This causes uneven wear and, in certain cases, mobility of one or more teeth. It can be corrected with orthodontics, but only if detected in time.
4. Dental trauma or an old injury
Sometimes, a forgotten childhood impact can cause progressive damage. I have seen teeth begin to move years after an accident because the tissue supporting them was already compromised.
5. Bone loss from other systemic causes
Conditions such as osteoporosis, uncontrolled diabetes, or autoimmune diseases can also weaken the bone supporting the teeth, causing them to loosen without an apparent cause in the mouth.
✋ Warning signs (even without pain):
You feel a tooth move when touching it or eating
You notice new gaps between your teeth
Your teeth no longer fit together as they used to
You feel pressure or discomfort when biting
You have constant gum bleeding
👩🏻⚕️ My message as a dentist in the Dominican Republic
I have seen tears in patients who lost a tooth while thinking it was nothing serious. That is why I write this with such conviction:if you feel a tooth moving, do not ignore it. It does not matter if it does not hurt.
At our clinic in the Distrito Nacional, we perform clinical and X-ray evaluations with advanced technology to determine the exact cause and act before it is too late. Every tooth can be saved if intervention happens in time.
