That painless cavity… could be silently destroying your smile
The story that repeats every day in my office

I am Dr. Sabrinsky Flores, and I have seen this pattern too many times: patients arrive calmly, without pain, and confidently tell me their teeth are fine. But during the evaluation, I discover an active, deep cavity in a tooth that appeared perfect. Then comes the surprise: “How can that be if it didn't hurt?”
The answer is simple and alarming at the same time: not all cavities hurt. Some progress for months or even years without you noticing, gradually destroying your tooth from within. And when it finally hurts… it is already too late.
Why does a cavity not hurt at first?
A cavity begins by attacking enamel, the outermost layer of the tooth. Enamel has no nerve endings, so even if there is a lesion, you will not feel pain. Only when the cavity reaches the dentin or nerve (the pulp) do you begin to notice sensitivity, discomfort… or that pain that no longer lets you sleep.
The most dangerous part is that when the cavity finally causes symptoms, the damage is already greater and treatment more costly and complex.
Painless cavities: what is happening in your mouth without you knowing?
Many believe that if there is no pain, there is no problem. But it is like a water leak behind a wall. You cannot see it or feel it… until the damage is already structural.
A silent cavity can:
🔴 Weaken enamel and cause tooth fractures
🔴 Infect the nerve, creating a need for root canal treatment
🔴 Cause abscesses or bone infections
🔴 Require extraction if the damage is irreversible
🔴 Trigger general health problems (infections, systemic inflammation)
How can a cavity be identified in time?
Even if it does not hurt, there are signs that can alert you:
✅ Dark or white spots on the tooth
✅ Persistent bad breath
✅ Mild sensitivity to cold or sweets
✅ Food that frequently gets trapped
✅ Changes in the tooth's texture
If you notice anything different in your mouth, however small, do not ignore it.
“I didn't feel anything”: what patients tell me every week
At our dental clinic in Santo Domingo, every week I see people surprised to have advanced cavities without having felt pain. Often they come for a cleaning, and we end up preventing an extraction thanks to early detection.
A patient once told me: “Doctor, if I hadn't had that evaluation, I would have lost the tooth.” And she was right.
Neglect is costly: consequences of waiting too long
⚠️ Acute pain
⚠️ Tooth loss
⚠️ Bone infection
⚠️ Complex and costly treatments
⚠️ Impact on facial aesthetics
⚠️ Effects on self-esteem and overall health
The emotional, cosmetic, and financial cost of a cavity not treated in time can be enormous. And the saddest part is that it often could have been prevented with a 30-minute appointment.
How do we prevent and treat cavities at my clinic?
🦷 Dental evaluations with modern technology
🦷 Digital X-rays to detect hidden cavities
🦷 Professional cleanings
🦷 Aesthetic, safe restorations
🦷 Personalized oral hygiene education
My approach is not only to treat, but to prevent. Your smile deserves care before something damages it.
How much does treating a silent cavity cost?
The estimate is confirmed after evaluating the tooth and determining the treatment needed. A filling and a treatment that includes root canal treatment, a crown, or tooth replacement have different scopes. Request an itemized proposal for your case; this article does not set prices or replace an evaluation.
Saving the cost of an evaluation today can be very costly tomorrow.
If you feel everything is fine in your mouth, I am glad. But if it has been more than six months since your last evaluation, I invite you to have one now. That painless cavity may be hidden, advancing relentlessly.
Your smile does not deserve to be destroyed by neglect. And I am here to help you protect it in time.
